Reports: Incident/Accident Records

You can create and download Incident and Accident Reports in OSHA format using Hera.

Creating Incident Records

To get started, hover your mouse over the Create menu at the top right of Hera. You will see the option to create an Incident Record. You also have the option to create an Accident Record.

That will bring you to the form below where you can fill out all of the pertinent information regarding this Incident/Accident.

Be sure to add all of your related Documents, Images, and Videos to this record.

Incident Types

General Incident and Accident are the default Incident Types. You can add custom Incident Types by clicking on the Incident Type box to reveal the dropdown menu and clicking the + Add New Custom Incident Type option.

That opens the drawer displayed below. To add a new type, click the + New Option button located below the list. Feel free to add as many new options as you need such as "Dog Bite."

*Note: You can also edit the Incident Type list from your Company Settings, in the Custom Lists section.

The columns give you a few actions explained below:

  1. OPTION: shows all your list options. You can reorder your list using the three lines located next to the option name. Click and hold, then drag to your desired spot. *Note: Options can be renamed as long as they are not being used.*

  2. USED FOR: a counter for how many incidents this option is assigned to. Options that are being used cannot be edited or deleted.

  3. ACTION: this column gives you two options. When the spinning arrows are displayed, clicking them allows you to assign the items in the USED FOR column to another existing option. The trash icon allows you to delete the option.

  4. STATUS: this column allows you to disable or enable options by clicking on the Disabled or Enabled pill icon. Disabled options will not appear as a dropdown option in their respective lists and cannot be assigned until they are enabled again.

View Incident Records

To view all of your Incident Records head to the Tasks & Reports page. You can scroll to the Associate Management section of the reports or use the search bar to find it faster.

That will take you to the list of your incident records. You will see key details of each record. Use the action menu (3 dots) to view the file. Use the filters at the top of the page to narrow down your results.

When viewing your incident forms you have the option to download, print, or delete the file via the icons on the top right.
